Waiver

This form can be filled out in person at our facility or print it, fill it out beforehand, and bring it with you!

Every participant must have one filled out before being allowed to participate.

Anyone 18+ can complete their own form.

Anyone 17 and under must have one filled out by a Parent or Legal Guardian, 

or a legal adult (that is present) willing to accept responsibility for underage participant.
